2005 Marina Monitoring and Tracking Survey
 Consent & Contact Information

Providing your email and zip code and submitting your answers to the surveys questions indicates that you voluntarily agree to participate in this survey. The time required to complete the study varies by the nature of your marina. We estimate between 15 and 30 minutes for the majority of marinas.

Information that you provide will remain strictly confidential and will only be used for research purposes. We will not release any information concerning your marina, or your email address, or your mailing address. "Your privacy will be protected to the maximum extent allowable by law." Since there are very rare situations in which a researcher may be compelled to break the confidentiality of subjects (e.g., in response to a subpoena or at the request of UCRIHS), no absolute guarantees to privacy are possible.

If you have any questions about this study, please contact Dr. Ed Mahoney (contact information below). If you have questions or concerns regarding your rights as a study participant, or are dissatisfied at any time with any aspect of this study, you may contact - anonymously, if you wish - Dr. Peter Vasilenko Chair of the University Committee on Research Involving Human Subjects (UCRIHS) Contact information provided below.
